Sub-Acute Case Manager

The  Actors Fund Home, an award-winning assisted living and skilled care facility in Englewood, NJ seeks a Subacute Case Manager who will be responsible for guiding patients and their families through the rehabilitation process.  The Case Manager will market to hospitals and referral sources, coordinate admissions to The Home's sub-acute facility, and finally, arrange and implement a safe discharge once services are completed.

 

ESSENTIAL DUTIES:

Coordinate admissions to The Home’s subacute

rehab unit:  50%

 

 

 

  • Liaise, market, and network with local hospitals and other referral sources to coordinate admissions. Develop relationships, provide excellent customer service, and educate referral sources on services provided by The Actors Fund. Maintain subacute census.
  • Travel to assigned hospitals to conduct on-site clinical assessments of potential patients and determine appropriateness for admission. Facilitate all aspects of a smooth transfer/admission process.
  • Utilize electronic health charting, and software such as American HealthTech, Epic, AllScripts, and other technology related to subacute admissions process.
  • Coordinate re-admissions of Actors Fund long term care residents when returning from the hospital.

 

 

Coordinate with IDCP team and department heads to provide exceptional health care services at The Actors Fund: 25%

 

 

 

 Community

 

  • Work with patients and families to complete admissions documentation once admitted.
  • Schedule Careplan Meetings with patients and families.
  • Participate in IDCP team meetings and Utilization Review.
  • Communicate with department heads and finance team regarding insurance and financial arrangements.

reintegration: 25%

  • Arrange and implement a safe discharge plan, to home or elsewhere, as developed by IDCP team.

 

OTHER DUTIES & RESPONSIBILITIES:

  •  May serve as Administrator on Duty as needed.
  • May be asked to cover for Director of Social Services or Admissions Social Worker.

KEY COMPETENCIES:

MINIMUM QUALIFICATIONS:

  • Bachelor’s Degree required, Masters preferred
  • Nursing or Social Work Degree preferred
  • Current New Jersey state license as appropriate
  • Excellent written, verbal, and interpersonal communication skills
  • At least five years’ experience in subacute and/or acute care
  • Experience with managed care/insurance, and public payer sources
